Waiver and Release; Delegation of Authority to Act in the Event of a Medical Emergency

Injury Waiver and Liability Release 

I acknowledge and assume the risk of injury to my child that is inherent in practicing for and in playing the game of baseball.  I hereby waive and release all Prep entities (Prep Baseball Report, Prep Baseball Report Multimedia, Prep Baseball Report & Partners, Prep Baseball Report Holdings, Prep Baseball Report Tournaments, and Prep Baseball Report State Affiliates) from all legal responsibilities and liability in the event of injury to my child while practicing or playing baseball at a “Prep Event”. 

Delegation of Authority to Act in the Event of a Medical Emergency

I know of no mental or physical problems which might affect my child's ability to safely participate in a Prep
Event.  Please list any of your child’s  health or medical conditions of which we should be aware:

Should there be an emergency requiring immediate medical attention and I am not immediately available,  l hereby authorize Prep Baseball to act for me so that my child will receive medical care as Prep Baseball deems necessary.

Waiver and Release of Name, Image, Likeness Rights

As a condition of participation in a Prep Baseball Event (whether a Prep Baseball Report and/or a licensee and/or an affiliate sponsored event), I hereby grant to Prep Baseball my full and complete permission and consent to use and disclose my child's name and registered contact information, including home address and telephone number, and his/her  picture, image, video, likeness, actions, voice, or other personally identifiable information (collectively “Name and Image”), in whole or in part, individually or in conjunction with other images, captured by any means as part of registration with and/or participation in any Prep Baseball Event .

I waive and release all rights of privacy or compensation which my child  has or may have in connection with their  Name and Image as so captured at the Event in any media whatsoever, to be used by Prep Baseball and/or its licensees, affiliates and/or sponsors for promotional purposes and for profit, without obligation to account to you or your child for any benefit from such use of their Name and Image. Such promotional purposes include, but are not limited to, allowing Prep Baseball website subscribers (such as college coaches and college athletic departments), to access, view and use your Name and Image for recruiting purposes. II consent to such uses for promotional purposes and hereby waive all rights to compensation and any right to inspect or approve the finished product or image, regardless of format.
